good luck with this one and sorry for taking so long to report it like i said i would.. i was having trouble making it happen again and i had to question if it was my guitar doing it since it needed cleaning recently but i am very sure now that it is not my guitar.. there is a bug that makes you miss seemingly randomly on a very specific pattern.. that pattern is a blue strum note and an orange hopo note.. the orange hopo note seems to have to be pretty close to the blue strum note in order for you to miss for no apparent reason.. what happens is when you strum the blue note sometimes it will not strum it.. this doesn't seem to happen for any other pattern and only for blue and orange.. it doesn't seem to happen even if it's the same pattern but instead using yellow and blue or red and yellow or green and red.. i think it worth mentioning that it seems to happen most near the beginning of a song (but this may be completely untrue.. when i miss i restart so there is more of a chance to miss near the beginning cause i am at the beginning more and such)

This seems quite a strange issue indeed I can't think of a reason why that would happen only in that situation. Does it happen if you use the keyboard??

i tried with the keyboard and it just wasn't happening (also i am not good with the keyboard so i couldn't do the same things i could on guitar.. i.e. hit everything up until i would be forced to miss) so i tried on the guitar again and boom it happened within a few runs.. so i tried on the guitar with Xpadder and removed the DL.ini from the game and turned SHOWPROFILEMESSAGE to FALSE and tried a good bit.. after about an hour of nonstop trying i'm pretty sure it happened.. i don't think that was my fault but i really wasn't paying close enough attention when it happened.. i have little reason to believe i would have actually missed though.. i will try more with the keyboard later but i was also having graphical problems.. the game looked perfectly fine and then all of a sudden it has extremely glitchy graphics.. it didn't seem to mess with the gameplay whatsoever though so i was able to ignore it and keep playing like usual but i thought it worth mentioning.. i'm thinking that is a codec issue (i had it before and fixed it already but don't remember how exactly).. either way i hope this doesn't give you too much trouble and sorry for more wall-o-text for relatively little information

OK, well any more info you find let me know. I find these bugs really hard as I suck with the guitar so I can never recreate them.

i will skip a TON of info that is probably pretty useless.. the more i tested stuff the less sense it seemed to make until my final bit of info which i will explain below.. if you want the full story i will try to give all the info i can remember but i think it's going to be way too long and not worth much if anything.. anyway this bug is tricky.. and it may very well be all my guitar's fault (it didn't seem to ever happen on the keyboard as far as i can remember) but i don't know that for sure.. so anyway i edited the track i had the most success with (in making the bug happen) and took out all the orange hopo's and played it like that and there were no problems.. but i tried that same edited track and pretended there were orange hopo's on every blue note i could including ones that didn't originally have any and the results were interesting.. turns out that makes me miss as well.. but why? i do not know.. i recorded me doing all that though and while playing it looks like i hold the right fret button down and strum and miss (with the overstrumming sound happening when i strum) for no apparent reason but when you look at the videos and pause at the right time it shows that for a very short time the fret button was not being held down or at least not registering.. i guess it was just long enough for the strum to miss.. i have only one idea of why this might happen.. i suppose when i move to press the orange note the controller could not detect the blue button being held down for a moment.. short enough where you can't see it easily but long enough to make me miss.. otherwise i don't know what it would be.. so i guess the real test comes when i get another guitar and try it instead.. i don't know when that will happen though..
so i suppose i will update more if i learn/think of anything else.. i still have to test this on a different game as well just to make sure but the good news is i already ran tests on another game and had similar problems so phase shift might be in the clear!
