There is GH import logic that adds drum roll phrases for drum notes that are long (ie. over 110ms) in the original chart file. At least this seemed to be how those games authored them. The lengths of the drum roll phrases when I import the MIDI you posted seem to show that almost all of the drum notes imported from the original chart file were pretty long (over 150ms). You may want to check with Bluzer on this, some charts may need to be altered after import if they don't follow the normal authoring conventions for that Guitar Hero format (ie. QB format).

Putting the EOF'd MIDI file back through EOF again let me see the special drums rolls and single drum rolls. I then edited the MIDI to correct these issues.
I think I had an outdated GHWOR download, so I'm redownloading the song to see if that's the case.
The song's tempo is fairly slow, so that's probably why EOF's logic for GH drum rolls was a bit confused. I don't have the original chart files on my computer right now, but I have a hunch this was sloppy charting on Activision's behalf.

EOF doesn't take the tempo into account for drum rolls, just the fact that the drum notes were long enough in milliseconds.

That would explain the the drum rolls. I loaded the original .pak.ngc and the notes do have "tails" to that of a 1/32 grid snap. So this is how I see how it happens. EOF loads the original file, finds that the notes have these "tails" then interprets them a special/drum rolls to the notes automatically(drum rolls in GH are similar to sustains). I blame JD! JKJK. But they're years old now(literally speaking), and updating them now might cause a mess with online compatibility. I know a guy(yb6|onemancoverband) whom I asked to re-work the audio for GHWoR and he is getting near completion. I can probably update midi's too when he is done so I will fix errors just like the one mentioned in OP.
