Well I'm back from... I guess nothing really. So if anybody does not remember, I am, in fact, the singer/guitarist/co-producer of this particular band, and so I hopefully will be able to contribute in the process.
For what it's worth, the bass is definitely a 4 string. Our bassist has a 5 string bass but didn't use it on this record. You can tell because his 5 string is fretted and his 4 string is fretless, and he used his fretless for the record.

Well I've already hit an impass, lol. For some reason, I can't play-test the latest version of Fragile Existence at all! When I open Phase Shift, it just immediately ends the song and gives me 0%, and then when I try to do it again, the game freezes.
And I can't import the chart into EOF without doing "Midi import". And as a result, weird chords are showing up where they shouldn't be. Like a 4 button chord, and the chord is always a blue/orange chord added inappropriately. I assume it's not the charting itself that's the problem.
Any way I can fix this apparent issue? Because I want to be able to chime in on what exactly the members of the band are doing, so that the chart can be as accurate (while still being fun) as possible.

I'll be off too soon to check anything, but I'll try it myself as soon as possible, and I will tell you.
I think a friend of mine already had this kind of problem. Make sure you have the very last version of EOF (1.8RC6), maybe there's a compatibility problem if you load the chart on a former version, who knows.

There shouldn't be any compatibility problem with the MIDI as long as you aren't using an old version where bugs were fixed.
bassbait, is there any particular reason why you couldn't open the project (.eof) file? It's in the first post, available with the "download" link. If you get an error about it being invalid when you open it, you must definitely be using an old version of the editor (ie. 1.7x, which is the version on EOF's Google Code website).
Using the latest version of EOF, I opened the project, then I imported the MIDI in another EOF instance. I did a quick comparison between the two and the MIDI import resulted in the correct number of notes for each track. I couldn't find any four note chords either. If you can reproduce a problem in EOF 1.8RC6 (http://www.dwsk.co.uk/index_phase_shift.html in the download section), please let me know.

I will let you know, I'm going to try that right now. But EOF doesn't change the fact that it's messing up in Phase Shift, so if anybody gets the same problem, or knows of a solution, it'd be greatly appreciated.

Ok, that fixed it in EOF. I have yet to try it in Phase Shift again (sorry, it's really late at night, so I can't be playing it right now), but I will when I get the chance.
Minor note (no pun intended) -
my guitar solo is charted a little wrong in a couple of parts. The first part is at 2:40 -
The P,B,R,P,B,R, pattern at 2:40? In the current chart, it's an 8th triplet pattern, but the actual guitar part is a 16th note pattern that is as such -
P,R,B,R,P,R,B,R,
It's hard to explain in GH terms but that's the best I can do.
At 2:53, there's a triplet pattern that's kind of missed.
That's really all there is. The chart looks pretty good otherwise!
As for the bass chart, there could be some improvements to be honest (no offense). There's a pattern at 1:13 that plays multiple times over the course of the song, and it's a little bit under-charted. And a few things here and there which could be touched up on a bit.
The chart that we have no goes like this -
O, Y,R,G, B-- Y--
But it should look more like this -
O, Y,R,G, B--R,Y--

Here are the fixes I tried to do from your explanations on guitar : 2:40 So it should look like this ? 2:53 Hearing the guitar in slowmo, that's what it looks like. I may replace the first yellow fret of the second triplet to a red one, but to me it sounds like it's the same note as before.

Oh, and don't mention the (RB) chord just after 2:40, it's just because of my mouse, there isn't any chord there
